There may not be too many clubs who run into the following issue, but for those that do, it can be a bit concerning. We have had a few reports of 'missing' pages of the form 8949; the club will have made entries in the tax printer listing cost basis figures from their 1099, and then get to the tax forms only to discover that none (some times only a handful) of the entries they made have appeared on the form 8949. In some cases, the form may not even show up.
The good news in this case is that it's due to a change in the tax forms this year. The change in wording and the layout of the form can be easy to overlook if you don't have the 2012 forms on hand for a reference. The change is on Schedule D, in part I and part II:
Totals for all short-term (long term in part II) transactions reported on Form 1099-B for which basis was reported to the IRS and for
which you have no adjustments
What this means is that at least on the 2013 tax forms, any place where the cost basis you enter from the broker matches the cost basis already in the tax printer, you do not need to make a separate entry on the form 8949.
